Please complete and submit all required forms two weeks prior to your Church Conference.

When attaching files in the forms, PDFs and Word DOCS are both accepted. Please put your city and church name at the top of any attached form. Thank you.


  • Every time you enter information in a field, it is saved to your session, which can be accessed by clicking on the Save and Continue button on the right side. A link will appear at the top of the page. ****It is important to highlight, copy and save this link for your records before exiting out.  You may paste this link in the address bar of your browser to re-enter your session at any time. ****
  • We recommend printing a copy of each submitted form to retain for your records. (Click on the “print button” before hitting submit. We recommend using a browser other than Internet Explorer such as Google Chrome or Firefox to be able to print.)
